HOW TO ORDER PRESCRIPTIONS
At Revival, we are committed to making the process of getting your pet's prescription easy, quick, and painless! Choose one of the four options below:
  • Have Revival Contact Your Vet

    You want us to contact your vet? Of course we will!

    Whether you call in or place an order online, all you need to do is request that we contact your vet when you fill out the Pet Care Form. If ordering online, this form will automatically pop up when you add a prescription item to your cart. If you are calling in your order, our Pet Care Pros will ask you a series of questions to complete this form. Once we have the information on this form we will contact your vet to get the prescription information.

    Important: Please make sure the information on the Pet Care Form is complete and accurate. This helps us get your prescription completed and shipped faster.

  • Have Your Vet Contact Us

    Can my vet call/fax Revival with the prescription? Absolutely!

    Whether you call in or place an order online, you'll need to fill out the Pet Care Form. If ordering online, this form will automatically pop up when you go to checkout. If you are calling in your order, our Pet Care Pros will ask you a series of questions to complete this form.

    After your order is placed, your veterinarian can fax the prescription to 877-335-2680 or call (712) 737-5555 Ext. 134. If our pharmacy staff is unavailable please leave a message. We can only accept faxed and phone-in prescriptions from a veterinarian.

  • Mail It To Us

    I have the original paper copy of the prescription. Can I mail it to you? Sure can!

    Whether you call in or place an order online, you'll need to fill out the Pet Care Form. If ordering online, this form will automatically pop up when you go to checkout. If you are calling in your order, our Pet Care Pro's will ask you a series of questions to complete this form.

    Once the order is placed, the original written prescription (no photocopies) can be mailed by either you or your vet to:

    Can I get a refill? Yep!

    If you need a refill, whether you call in or place an order online, you'll need to fill out the Pet Care Form. For refills be sure to include the refill number on this form. If you can't find your refill prescription number, don't worry we can find it for you.

    If ordering online, the Pet Care Form will automatically pop up when you go to checkout. If you are calling in your order, our Pet Care Pros will ask you a series of questions to complete this form.

  • Transfer Your Prescription

    Have a prescription you'd like to transfer? No problem!

    Locate the prescription item you'd like to purchase and add it to your cart. Then just fill out the Pet Care Form that pops up, including the contact information for your current pharmacy, and place your order. Our Pharmacy Team will contact your pharmacy about the transfer. If no transfer is available, we'll reach out to your vet based on the information you provide in the Pet Care Form.

  • How do I get prescriptions to the Revival pharmacy?

    There are three easy options to get your prescription to us.
    1. We would be happy to contact your vet for you.
    2. Have your vet contact us.
    3. Mail the Revival pharmacy the original written prescription.

  • Where is my order?

    Our Pet Care Pros would be happy to determine where your order is in the process. If a prescription is needed, sometimes it takes a few days for a veterinarian to get back to us. It may help to contact your veterinarian as well to check on when the prescription will be sent to the Revival pharmacy. Feel free to call our PCPs to check on your order status at 1-800-786-4751.

  • My veterinarian called in my prescription. Why do I not have my order?

    Sometimes prescriptions will come into the pharmacy but need an order to progress. If you have placed an order, chances are it is being processed by our pharmacy staff. Orders are worked on in the order received.

    There's also a possibility we didn't receive the prescription from the veterinarian for a variety of reasons. Always feel free to call our friendly Pet Care Pros at 1-800-786-4751 to track your order.

  • Why is my prescription taking so long to ship?

    The most common reason for prescription order hold ups is waiting for the veterinarian to authorize the prescription. Veterinarians can be very busy with surgeries and other clinic activities and sometimes don't get to prescription requests right away. Another issue that we commonly see is when a pet is listed at the veterinarian under another owner name so when we call to request the prescription, the clinic cannot find the animal in their system. This can cause a hold up.

    Don't hesitate to call our Pet Care Pros at 1-800-786-4751 to find out the current status of your prescription. They will be able to tell you if the order has already shipped or if the carrier is having an issue with shipping. Our team is happy to help!

  • How come my veterinarian hasn't been communicated with?

    Revival makes every effort to contact your veterinarian as quickly as possible and follow up on prescription requests. If you have changed your veterinary clinic be sure that you've updated the information we have on file.

    If you have any questions as to why your vet clinic hasn't been contacted please call our friendly Pet Care Pros at 1-800-786-4751. They can review the order notes with you to see if there is a reason for the delay.

  • Why is shipping so high on such a small prescription item?

    The rate schedules are negotiated contracts with our carriers. Our Package Care Pros work to keep the costs down.

  • How come my prescription item is not in a stock package?

    The Poison Prevention Act requires safety lids on all prescription bottles to help prevent poisoning in children. If we receive a stock package without a safety lid, we then repackage it in prescriptions bottles with safety packaging. The lot number and expiration dates are recorded on the repackaged bottle and a package insert is kept with the safety bottle. These prescriptions are then all checked by our pharmacists.

  • Why are my veterinarian's instructions different than the label?

    It's best to contact your veterinarian for clarification on the correct directions. The pharmacy places directions on the prescription label based on what the prescription reads. Sometimes the veterinarian changes directions during the course of the prescription; however, refills still reflect what the original prescription said. To remedy the situation, have the veterinarian give us a new prescription or let the Revival pharmacy know and we can request corrected information.

  • Why do I sometimes get voicemail when I call the Revival Pharmacy?

    Our pharmacy staff sometimes relies on a voicemail to take calls for us if we are filling orders or experiencing high call volume. The voicemail is checked often and it is a specialized, secure voicemail for veterinarians to leave prescriptions. If you have a concern that a Pet Care Pro is unable to help you with, please leave a message and a member of our pharmacy staff will call you back as soon as possible.

    There are also times when we experience a high call volume and all of our pharmacy staff are on the phone helping callers. At these times, please leave a message and we will call you back.

  • Why didn't someone tell me I needed a prescription for this item?

    Revival makes every effort to show if an item in the catalog or online is a prescription item. There will be a red Rx symbol next to the product and text indicating a prescription is required. This is an example of the symbol used stating that an item is in need of a prescription.RX logo

    When a prescription item is put into a cart online it brings up a pet information sheet that states that a prescription is needed and asks for your veterinarian's information. It's also important to note that some states have different regulations and laws regarding prescriptions. For example, some states require a prescription for a certain medication while another state does not. In those cases, there is a note next to the item that shows a prescription is needed for certain states.
